Abstract: 

در ایالات متحده آمریکا، تروما شایع ترین علت مرگ و میر زیر 40 سال می باشد و حدود نیمی از مرگ و میر آن، مربوط به ضربه سر است. در بیمار با آسیب دیدگی حاد، توجه به راه هوایی، علایم حیاتی، قفسه سینه، خونریزی و بی حرکتی ستون فقرات، قبل از مغز اهمیت دارد.بازآموزی مواجهه با ضربه متعدد و ضربه سر برای کلیه کارآموزان، کارورزان، پزشکان عمومی، دستیاران و متخصصین رشته های جراحی توصیه می شود.اهداف مقاله:1- افزایش آگاهی به اهمیت راه هوایی، تنفس و گردش خون در تروما.2- مخاطب بتواند نحوه برخورد به آسیب راه هوایی، تنفس و گردش خون در تروما را بیان کند.3- افزایش آگاهی به جایگاه جراحی عمومی و جراحی مغز و اعصاب در تروما.4- مخاطب بتواند نحوه برخورد به آسیب دیدگی های مغز را بیان کند.با مطالعه دقیق این مقاله، مخاطب باید بتواند به حداقل 80 درصد سوالات مطرح شده پاسخ درست بدهد.

Abstract: 

Background: We aimed to investigate the clinical value of detecting 14 food intolerances. Methods: A total of 312 patients with food intolerance enrolled in the Third Affiliated Hospital of Qiqihar Medical University (Qiqihar, China) from Feb 2016 to Feb 2017 were selected. ELISA was used to detect intol-erance specific IgG antibodies for 14 foods (pork, chicken, beef, shrimp, fish, crab, egg white/yolk, tomato, mushroom, milk, corn, rice, soybean, wheat). Results: The highest average positive rate of the patients was 42. 31% for the crab, followed by shrimp 21. 15%, egg white/yolk 18. 27% and milk 16. 99%. The positive rate from high to low was crab, shrimp, egg white/yolk, milk, fish, corn, soybeans, tomatoes, rice, mushrooms, wheat, pork, beef, chicken. There were significant differ-ences in the specific IgG antibody positive rates between shrimp, soybean and wheat in the skin symptoms group, gastrointestinal symptoms group, respiratory symptom group and nervous system symptom group (P<0. 05). There was a significant difference in the positive rate of specific IgG antibodies between shrimp, crab and egg white/yolk in the adolescent group, the middle-aged group and the elderly group (P<0. 05). Conclusion: The detection of food-specific IgG antibodies can help to determine which food intolerance caused the disease, and then adopt a fasting or diet method to avoid eating unsuitable foods and continually damaging the body, thus maintaining good health. The detection method provides a new idea for the diagnosis and prevention of diseases.

Abstract: 

The result showed the maximum resistance for imipenem (100%), meropenem (100%), cefotaxime (98.6%), and ceftazidime (98.6%), and minimum resistance for tetracycline (65.7%) and gentamicin (68.6%). The MCBT test for meropenem-gentamicin-tetracycline antibiotics demonstrated 65.6% and 6.3% of MDR Acinetobacter baumannii strains killed at high and low concentrations of antibiotics, respectively (P=0.04). Besides, 34.4% and 93.8% of MDR Acinetobacter baumannii strains lived at high and low concentrations of meropenem-gentamicin-tetracycline antibiotics, respectively, which were statistically significant (P=0.04). There was a significant relationship between the MIC of gentamicin-meropenem in the combination (Checkerboard) (P=0.0001). Antibiotic interaction effects frequencies were 6.3%, 53.1%, and 40.6% for synergy, incremental, and indifference, respectively. In this study, a significant difference was shown between the FICIs of meropenem (0.68±0.27) and gentamicin (0.003±0.003) (P=0.0001). Also, there was a significant correlation between the FICI factor and the antibiotic's antimicrobial interaction by MCBT (r=0.95, P=0.0001).Conclusion: It seems that the combination of meropenem-gentamicin-tetracycline antibiotics had additive antibacterial effects and could be used to suppress MDR Acinetobacter baumannii isolated.

Abstract: 

Purpose: To report a patient with MEWDS.Patient and findings: A 65-year-old man presented with sudden decrease of vision in his right eye. On ophthalmoscopy of the involved eye, numerous small discrete white lesions were detected superficially and deep in the retina around the disc and macula. On visual field examination, enlargement of the blind spot was detected. Fluorescein angiography was unremarkable. On electroretinography, the a-wave was decreased minimally. The patient was followed and after 2 months, significant improvement of visual acuity and visual field was observed.Conclusion: MEWDS is a rare intraocular inflammatory disease of unknown etiology and in contrast to our patient, more frequent in females. The natural course is generally benign with spontaneous disappearance of the lesions and improvement of vision in 6 to 8 weeks.
